Patient Information

First Visit

Your initial appointment will start with a new patient examination and explanation of the diagnosis and treatment options. If needed, a hygiene session will also be recommended.

At the time of booking, let the office know if you have an immediate dental need—such as a broken tooth/filling, pain, or swelling. That way we can accommodate you and provide a specific treatment the same day as your examination.

More complex treatment plans can be delivered over a period of time to achieve the desired results with respect to your time and financial considerations.

Please assist us by providing the following information for your first visit:

  • If your previous dentist has taken any recent X-rays (radiographs), please have them forward the X-rays to our office prior to your appointment OR bring them with you. If your X-rays are not up-to-date, we will take new ones.
  • A list of medications you are presently taking. Let us know if you require antibiotic coverage prior to dental treatment.
  • If you have any dental insurance (primary, secondary or both), bring all the necessary information so we can process your claim in a timely manner.

We value your time, and we pride ourselves on keeping appointments on schedule. Accordingly, if you need to cancel or reschedule your appointment, we respectfully ask that you notify us 48 hours in advance so that another patient in need can be seen in your place.

For your convenience, you can download the new patient/dental forms to expedite the paper work involved. If you prefer to fill them out at our office, please arrive 10-15 minutes prior to your scheduled appointment.
